Health and Wellness Programs

Blue Door Neighborhood CenterSM

Blue Door Neighborhood Centers (BDNCs) are community hubs. They are free and available to all. BDNCs provide face-to-face engagement with Care Coordination. They also offer health education and health and wellness programs. BDNCs also help you access community resources. Visit one of the BDNCs in the Pullman Morgan Park  and South Lawndale  neighborhoods.

Care Van® Program

Our Care Vans help build healthy communities. Some of the services they give are:

  • No cost immunizations
  • Dental services
  • Health literacy education
  • HIV testing
  • COVID-19 testing

Learn to Live*: Digital Mental Health Platform

Learn to Live is a no cost online health program. It is offered to members 13 and older and caregivers. It gives self-paced mental health solutions. Plus, access to 24/7 member coaches. It can help with common challenges like:

  • Stress
  • Anxiety
  • Depression
  • Insomnia
  • Substance abuse

Preventive Care

Keeping up with health screenings can be hard. That’s why we are here to help. 

Telehealth

BCCHP has made it simpler to see your provider. We do this by offering Telehealth choices. This way, you can get the care you need. This includes virtual care.

Value-Added Benefits (VABs)

We can help you decide when to get screenings and vaccines. You can also get incentives for completing some of these screenings. [PH1] Talk to your doctor about any health screenings you might need.

Social Determinants (SDoH)

Did you know that where you live or work can have an impact on your health? BCCHP has a list of SDoHs that can impact your health. BCCHP also helps you learn how to stay healthy even if you are affected!
