Policy Documents Overview

In some cases, your Blue Cross and Blue Shield of Illinois policy document may be available without logging in to your secure Blue Access for Members account or using the BCBSIL app.

Help locating your policy

  • Get your policy documents through your online account .
  • Get your policy documents through the BCBSIL app. Learn more about the BCBSIL app
  • Contact a customer service representative at 1-800-538-8833 to get help finding or accessing your policy documents.
  • If you have coverage through an employer, you may want to contact the employer's benefit administrator to get the name of your plan.
  • If one of the following products or plan types sounds like your health care coverage, go to that page to find your plan and download its policy.

Small Group Affordable Care Act Policy Documents

  • You enrolled in a health plan through an employer,
  • The employer has between 1 and 50 employees, and
  • Your plan is a "metallic" (platinum, gold, silver or bronze) health plan.

Small Group Pre-Affordable Care Act Policy Documents

  • You enrolled in a health plan through an employer,
  • Your plan was first effective before January 1, 2014,
  • The employer has between 1 and 50 employees, and
  • Your plan is not a "metallic" (platinum, gold, silver or bronze) health plan.

Mid-Market Group Policy Documents

  • You enrolled in a health plan through an employer,
  • The employer has between 51 and 150 employees, 
  • You have a fully-funded health plan  and
  • Your plan is not a "metallic" (platinum, gold, silver or bronze) health plan.

Blue Balance Funded Policy Documents

  • You enrolled in a health plan through an employer,
  • The employer has between 5 and 150 employees, 
  • You have a self-funded health plan  and
  • Your plan is not a "metallic" (platinum, gold, silver or bronze) health plan.

Individual Pre-Affordable Care Act Policy Documents

  • You enrolled in a health plan on your own (instead of through an employer),
  • Your plan was first effective before January 1, 2014, 
  • Your policy is a grandfathered or transitional plan, and
  • Your plan is not a "metallic" (platinum, gold, silver or bronze) health plan.
