Develop a Quote

Preparing for a Quote

Use the "Quote a Group" eSales tool in your Blue Access for ProducersSM account

Get the Smart Census Import Tool Guide  for quoting steps. Navigate to the Middle Market Quoting Census Template section for step-by-step instructions.

Get New Sales Forms

Go to Form Finder. Click on Mid-Market Group from the left menu. You'll need to complete the following:

Benefit Program Application (BPA)
Use a BPA as the employer and account-level application for group coverage.

Benefit Plan Selection Form (BPS)
Use a BPS to identify medical, dental and life plan selections.

Employer Group Information (EGI)
Submit required documentation on group size, MSP and COBRA status.

Submit an Enrollment Request

If you normally work with a General Agent, please continue to send paperwork and inquiries to your GA, as usual.

Log in to your Blue Access for Producers account.

Review Current Coverage Documents

Get Renewal Forms

Go to Form Finder. Click on Mid-Market Group from the left menu. You'll need to complete the following:

Benefit Program Application (BPA) for Mid-Market Groups
Use a BPA to update employer information, change eligibility requirements or adjust premium periods.

Benefit Plan Selection Form (BPS) for Mid-Market Groups
Change, add or renew medical, dental and life plans. A BPS isn't needed if a group renews without changes.

Group Enrollment Application/Change Form
Use this form to enroll new members or update existing memberships and plan selections.

Employer Group Information (EGI)
Submit required documentation on group size, MSP and COBRA status.

Submit an Enrollment Request

If you normally work with a General Agent, please continue to send paperwork and inquiries to your GA, as usual.

To submit BPA (account-level changes) or BPS (plan changes) forms for renewing groups:

  1. Log in to your Blue Access for Producers account.
  2. Submit changes via the eSales Request Center tool.
