How to Enroll

If you qualify for Medicare and Medicaid, the Blue Cross Community MMAI (Medicare-Medicaid Plan)SM may be a good choice for you.

Who is this program for?

The MMAI is for people who meet all of the following requirements:

  • Live in any county in Illinois
  • Have both Medicare Part A and Medicare Part B, and
  • Are eligible for Medicaid, and
  • Are a United States citizen or are lawfully present in the United States, and
  • Are age 21 and older when you enroll, and
  • Are enrolled in the Medicaid Aid to the Aged, Blind and Disabled category of assistance, and
  • If you meet all other Demonstration criteria and are in one of these Medicaid 1915(c) waivers:
    • Persons who are Elderly
    • Persons with Disabilities;
    • Persons with HIV/AIDS;
    • Persons with Brain Injury;
    • Persons residing in Supportive Living Facilities

You can enroll online at the Illinois Department of Healthcare and Family Services.

By Phone

You can call Monday — Friday, 8 a.m. to 6 p.m. The call is free.

By Mail (Enrollment Form)

You may receive an enrollment form in the mail. Fill out this form and return it to:
