Is Managed Long-Term Services and Supports (MLTSS) Right for Me?

If you qualify for Medicare and Medicaid, you may be eligible for HealthChoice Illinois MLTSS. Once you enroll in HealthChoice Illinois, you can choose to become part of our plan. The Blue Cross Community Health Plans℠ MLTSS plan helps you with your long-term care. Long-term care helps you do everyday things you can't do for yourself.

How Do I Join?

Joining is easy. There are two ways for you to join Illinois Medicaid. Call or go online. Illinois Client Enrollment Services will send you more information. This will help you choose a health plan when you are approved.

Am I Able to Enroll?

To check if you can apply to HealthChoice Illinois, go to the ABE website.

You may be eligible for MLTSS coverage if:

  • You are eligible for Medicare Part A and Medicare Part B
  • You are eligible for Medicaid
  • You are age 21 and older at the time of enrollment
  • You receive the Medicaid Aid to the Aged, Blind and Disabled type of assistance
  • You do not have End-Stage Renal Disease (ESRD). There are some exceptions to this. They include:
    • If you develop ESRD when you are already a member of a plan that we offer.
    • If you were a member of a different plan that was terminated.
  • You meet all the above and need services from one of these Medicaid 1915(c) waivers:
    • Persons who are Elderly
    • Persons with Disabilities
    • Persons with HIV/AIDS
    • Persons with Brain Injury
    • Persons in the Supportive Living Program
