Nursing Facilities

Do you have health care needs that can’t be managed at home or in an assisted living setting? You may need to live in a Nursing Facility (NF). NFs are sometimes called Nursing Homes or Long-Term Care Facilities. They provide short- or long-term care services to those with physical and/or mental disabilities. NFs are for the elderly and young adults. These services may need prior authorization from your health plan.

Nursing Facility Services

Personal Care

In an NF, you can get help with personal care activities, such as:

  • Eating
  • Walking
  • Bathing
  • Using the bathroom
  • Laundry
  • Meals
  • Housekeeping needs
  • Medical care

Medical Care 

Based on your needs and level of care, your stay in an NF will be determined to be skilled or custodial. Medical services can include:

  • Medically complex care
  • Rehabilitation (physical, occupation and/or speech)
  • Wound care
  • IV Therapy

Specialized Programs

Many NFs offer specialized programs for: 

  • Congestive heart failure
  • Complex wounds
  • Pain management
  • Alzheimer’s care
  • Respite care 
  • Other illnesses and conditions

If you live in an NF and would like to move to a different setting, our team of care coordinators can assist you. They can help you plan and prepare for a move through our Community Transition Program.
