You have the option to allow health information that we maintain about you to be accessed through a third-party online application (web app). If you agree to share your personal information with a third-party app, you can manage your consent at any time by using the Consent Management Portal.

This notice provides helpful information about who is eligible to access certain types of data through an online application (web app) that you use.
