Action Required: Updated Summary of Benefits Issued for Select Medicare Advantage Plans

We’ve updated Summary of Benefits (SOB) documents for select Medicare Advantage plans to reflect the correct $750 comprehensive dental allowance (previously listed as $1,000). Refreshed SOBs are ready to download.

Plans with updated SOBs:

  • Blue Cross Medicare Advantage Basic Plus (HMO)℠ (H3822-007)
    • English stock ID: 248462.1025
    • Spanish stock ID: 249184.1025
  • Blue Cross Medicare Advantage Choice Plus (PPO)℠ (H8634-003)
    • English stock ID: 248926.1025

    • English stock ID: 243088.1025

What You Need to Do

Only the SOBs for the affected plans named above need to be replaced. All other materials remain accurate and do not require action.

Action items:

  • Download refreshed SOBs from the Medicare Advantage digital enrollment kit or the BCBSIL Medicare website. Visit the Producer Supply Portal  to access the digital enrollment kit. If you received print enrollment kits before Oct. 22, 2025, new copies will be mailed the week of Nov. 11
  • Replace all older versions downloaded or printed before Oct. 22 with the updated SOBs.
  • Remove outdated SOBs for affected plans from your distribution. Outdated versions have a stock ID ending in .0725.
  • Verify you have the correct version by checking the stock ID in the bottom right corner of the document. Current versions end in .1025.
