Your Clients Received a Short Health Survey: Member FAQs Included
Aug. 26, 2026
Applies to Individual & Family Markets
Helping clients stay connected to their health coverage goes beyond enrollment. That’s why we’ve sent members an email inviting them to complete a brief health check-in through their online member portal.
The check-in consists of questions designed to better understand members’ health needs and connect them with available programs, benefits and support.
Your clients may contact you with questions after receiving the email. Use it as an opportunity to:
- Encourage them to complete the health check-in
- Remind them to log in to their online member portal
- Reinforce the value of the programs and resources available through their coverage
A simple conversation can help clients stay engaged with their plan while reinforcing your role as their trusted advisor. Members who use their benefits are more likely to renew. Below are the FAQs that members will receive along with the survey.
Health Check-In FAQ
Q: Why should I take the health check-in?
A: The health check-in helps Blue Cross and Blue Shield of Illinois understand your needs so they can suggest benefits, programs and resources that may be helpful to you. In some cases, a health professional may reach out to offer support, answer questions or help connect you with care that fits your situation.
Q: Can I earn rewards or points for taking the health check-in?
A: No. At this time, BCBSIL does not offer rewards or points for completing the health check-in.
Q: How is this different from the health assessment in Well onTarget®?
A: The health check-in is a short, simple questionnaire that helps us identify ways to support you right now. The Well onTarget health assessment is more detailed and provides deeper insights into your overall health and wellness.
Q: What does BCBSIL gain from the health check-in?
A: Our goal is to help our members stay healthy and get the most from their benefits. The health check-in helps us connect you with the right resources, support and care options. This can lead to better health outcomes and may help lower health care costs.
Q: How are my answers used?
A: Your answers help us understand what information, tools and support may be most helpful to you.
For example:
- If you've recently been in the emergency room or hospital, we may offer support for follow-up care.
- If you're unsure where to go for care or who your primary care provider is, we may share resources to help.
We use your answers to connect you with relevant benefits, programs and services.
Q: Will my answers affect my benefits?
A: No. Your answers will never change your benefits. They are only used to help connect you with benefits, resources, and support that are already available through your health plan.
Q: Will my answers affect what I pay for my coverage?
A: No. Your answers will not change your monthly premium, copays, deductibles or other costs. However, we may share information about programs or care options that could help you save money.
Q: Are my answers secure?
A: Yes. We take your privacy seriously and protect your personal information in accordance with our privacy and security policies.
Q: Can my dependent take the health check-in?
A: Yes. Adult dependents, such as a spouse, partner or adult child, can take the health check-in if they have their own BCBSIL online account.
Q: Can I complete the health check-in for my spouse or partner?
A: We recommend that each adult complete their own health check-in. The questions are personal, and the responses should reflect each person's own health situation and experiences.
Q: Can I skip questions I'm uncomfortable with?
A: Yes. The health check-in is voluntary, and you can skip any questions you do not want to answer. However, the more information you choose to share, the better we can connect you with benefits, resources and support that may be helpful to you.
