Start using the updated Health Off-Exchange and Dental Off-Exchange fillable paper applications. They are now available in the IFM Sales Tool Kit. The updates replaced the link BlueDentalInfoIL-2026 with BlueDentalInfoIL2026.

Stop using the previous versions of paper applications for any Special Enrollment Period and begin using the updated forms moving forward. You will know it’s the new version of the form if you see a “0326” on the bottom right-hand side of the application.
