Vision Coverage
Your Health Benefits for Immigrant Adults and Seniors (HBIA/HBIS) plan includes vision care.1 Blue Cross and Blue Shield of Illinois has partnered with Heritage Vision Plans, Inc., powered by VSP®2. We want to help you get the vision care you need. The plan includes routine vision care, eyeglasses, and eye check-ups.
What is Covered?
To make sure your vision care is covered, go to an in-network provider. You do not need approval from your PCP for vision care.
Some vision care requires approval before you get treatment. Without approval for the service, you may have to pay. To learn more, check your Member Handbook. Or call Member Services.
Eye Exams
- One eye exam every 12 months per member
Eyeglasses
- Every two years for member age 21 and older
- Replaced "as needed" for members under 21
- Every two years get $40 toward a pair of upgraded eyeglass frames
Contact Lenses
- Contact lenses when medically necessary if glasses cannot provide the intended result
What Is Not Covered?
The plan does not cover:
- Contact lens insurance
- Low vision aids
- Laser vision correction
