PPO Benefits

HSA Value and HSA Advantage

You may contribute to a Health Savings Account (HSA) to help pay for eligible out-of-pocket health care expenses tax-free.
You have access to the PPO network of providers. Find a doctor or hospital.
You pay the negotiated amount for all services (doctor’s office visits, prescriptions, hospital visits) until you meet your individual deductible.

Copay Value and Copay Advantage

You pay a flat dollar amount (i.e. copay) for certain covered services.
Deductibles and coinsurance still apply for many services, including certain diagnostic and treatment services performed in a doctor’s office setting.
Copays are applied towards your out-of-pocket maximum, not your deductible.

Prescription Drug Benefits

Your prescription coverage is provided by a third-party pharmacy benefits manager. Contact your company’s pharmacy benefits.
