Blue Advantage HMO provides valuable benefits, member services and flexibility, along with the security of predictable copayments so there are no financial surprises.

• You have access to certain doctors and hospitals, called your HMO provider network that helps control how much you pay for health care.
• Your care is managed by one primary care physician — your personal doctor — who helps make sure you get the right care at the right time and at the right place.
• Your prescription drug benefits are available through Prime Therapeutics^®.

When you join Blue Advantage HMO, you must choose a contracting medical group within your network. You will also need to choose a primary care physician (PCP). Your PCP provides or coordinates your health care, helps you make informed decisions and, when necessary, makes referrals to specialists who are usually within your medical group network. Each covered family member can choose a different medical group and PCP from the network.
