The Illinois Department of Healthcare and Family Services has contracted with BigHeart Health to operate the Medicaid Preventive Care Education Organization program, My Health. My Power. As part of this HFS-sponsored preventive care initiative, BigHeart Health is reaching out to eligible Illinois Medicaid members to enroll them in the program.
What this means for you: When gaps in care are identified, BigHeart Health will coordinate with in-network providers to help schedule wellness visits and other preventive services. This effort will support Healthcare Effectiveness Data and Information Set (HEDIS®) measures to address unmet care needs.
The goal of the MPCEO program is to educate, engage and support more than 500,000 Illinois Medicaid members by:
- Increasing awareness of preventive health services
- Identifying gaps in care
- Coordinating care plans with providers
- Connecting members to in-network primary care and preventive services
Outreach efforts will focus on three priority regions:
- South Suburban Cook and Will County
- Central Illinois
- Southern Illinois
