The Illinois Association of Medicaid Health Plans has released an updated Universal Roster template, effective Feb. 1, 2026. We accept this roster for provider information verification and updates.
The template includes added data elements related to race, ethnicity and telehealth capabilities. These fields are required for Medicaid providers. All new fields will be published in our provider directory.
