Managed Long-Term Services and Supports Transition for Blue Cross and Blue Shield of Illinois

Oct. 13, 2026

Effective Jan. 1, 2027, MLTSS services will transition from the HealthChoice Illinois program to Prepaid Inpatient Health Plans operated by a Fully Integrated Dual Eligible Special Needs Plan sponsor. As part of this transition, BCBSIL will no longer provide coverage for MLTSS members, effective Jan. 1, 2027.

What's changing?
Beginning Jan. 1, 2027:

  • MLTSS members will receive their MLTSS benefits through one of the awarded FIDE SNP sponsors operating as an MLTSS PIHP.
  • All current MLTSS members with BCBSIL will be reassigned by the Illinois Department of Healthcare and Family Services to one of the approved MLTSS PIHPs.

What’s not changing?

  • Coverage and administration of Home and Community-Based Services waiver services for our non-MLTSS waiver members with Blue Cross Community Health PlansSM aren’t changing.
  • For the full list of HCBS services, refer to the BCCHPSM provider manual.

Provider action needed
You should:

  1. Review your current MLTSS patient populations to identify members who may be affected by the transition
  2. Monitor communications from BCBSIL, HFS and receiving MLTSS PIHPs regarding member assignments and continuity–of-care requirements
  3. Assist affected members with understanding plan changes and direct them to the appropriate health plan resources as transition information becomes available

BCBSIL will coordinate continuity of care with receiving MLTSS PIHPs as permitted under the Health Insurance Portability and Accountability Act of 1996 treatment, payment and operations provisions to support a smooth transition of care.
