International Health Claim Form

1.       You May Not Need to File a Claim: Many Hospitals Will File Directly with World Access

Please show your World Access/Blue Cross and Blue Shield identification card packet to the hospital. If you are filing a claim, please fill out the International Form. Help us avoid unnecessary delays by answering all the questions completely.

2.       Help Us Process Your Claims Quickly: Insist on Itemized Bills

We want to process your claims quickly, but we can't do so without properly itemized bills. Attach Original Bills to the claim form. We recommend that you make copies of each bill for your personal records. The original bills will not be returned. 

3. Be sure to use to attach all documentation that you receive from the provider
