Do I have to receive care by in-network doctors and health care professionals?
If you have an HMO plan, you must seek in-network care for your benefits to apply, except in emergencies.
In the case of a life-threatening illness or injury, go to the nearest emergency facility right away. Emergency services are covered at your in-network benefit level. How do I find a new in-network HMO medical group or independent practice?
We can help you find an in-network HMO medical group or independent practice. You can start the process by:
- Visiting Find Care
- Registering for or logging in to BAM℠. Then follow the steps to change your medical group or independent practice
- Calling the number on your member ID card. Select “medical site changes” to use the automated system. Or choose “all other inquiries” to talk to Member Services.
Please select an HMO medical group or independent practice in the HMOI network by May 31, 2025. After you choose your group or independent practice, you will have the opportunity to select a primary care provider.
If you don’t make your selections by May 31, we will choose an in-network group for you, effective July 1. You will receive a new member ID card with your new group’s details. You can change your PCP and group selection at any time.
To change your medical group, log in to BAM, or call the number on the back of your member ID card.

What if my APP doctor or health care professional is affiliated with another HMO medical group or independent practice in the HMOI network?
Your provider may be affiliated with other HMO medical groups or independent practices that are in-network. If they are, you may select that group or practice to continue seeing your provider. Check our online Provider Finder® to see if they are affiliated with any other in-network HMO group or independent practice, or call the number on your member ID card.

When APP is no longer in network, will I have to change doctors?
If you have an HMO plan, you must seek in-network care for your benefits to apply, except in emergencies. Check if your doctor or health care professional is affiliated with any other HMO medical group or independent practice in the HMOI network. Use Provider Finder or call the number on your member ID card.

What if I am pregnant or being treated for a medical condition by APP?
If you are pregnant or being treated for a disability, acute condition or life-threatening illness, you may be eligible for continuity-of-care benefits. This means you may still be able to receive care for a period of time by your APP doctor and health care professional at in-network rates even when they are no longer in your network. Call the number on the back of your member ID card to find out if you are eligible. Your doctor may also start the continuity-of-care request by calling the prior authorization number on your member ID card.
You may start the process by completing a Request for Continued Access to Providers form or by calling the number on your member ID card. Forms should be faxed to the number on the form. Once reviewed, you and/or your doctor may be contacted for more information, like medical records. A decision will be made and shared with you or your doctor.

Will Advocate Health facilities still be in-network?
Advocate Health facilities are not impacted and are still part of the HMOI network. If you have questions, call the number on your member ID card. Our customer advocates will help answer questions about your specific benefits and provide guidance.
