Blue Choice Select PPOSM

  • PPO Plan: 9.7% Cost Savings relative to broad PPO2
  • Provides access to a narrow network of contracting physicians and hospitals in Illinois at an affordable cost
  • Depending on the structure of your client's benefit design, this selection could potentially decrease their health care costs and their employees’ overall health care spending

Available throughout Illinois except for Sangamon, Lawrence and Wabash counties.

Blue Choice OptionsSM

  • Tiered PPO Plan: 8.2% Cost Savings relative to broad PPO3
  • Leverages benefit design to increase utilization at cost-effective providers
  • A lower-cost alternative to the broad PPO. There are three benefit tiers:
    • Tier 1 – Blue Choice Options PPO. Lower cost-share when employees choose from this select network of providers
    • Tier 2 – Broad PPO. The flexibility of accessing the broader PPO network, but with a higher cost-share than Tier 1
    • Tier 3: – Out of Network. The ability to use OON providers and still receive some coverage

Savings will differ based on each employer’s population (demographics and location).
