Unless otherwise noted on the form, please send completed forms with any required documentation to:

  • Medical Claim Form – Use this claim form to request reimbursement for applicable medical expenses incurred for services not directly billed to the plan.
  • Transition of Care Form – Use this form if you are currently receiving medical care from providers that are not listed in your provider directory and would like assistance in coordinating your medical care with the new medical plan.
  • Disabled Dependent Certification – Use this form to certify a covered dependent's disability status for the purpose of continuing care past the plan's maximum dependent age when your dependent has been medically certified as disabled.
