If you are living with ongoing health concerns, Blue Cross and Blue Shield of Illinois (BCBSIL) offers one-on-one nurse support through the Primary Nurse Team. Personal support is available to you at no additional cost.

If you have an ongoing health condition, a BCBSIL nurse may reach out to you by phone. It is up to you whether you want to work with a nurse. If you choose to do so, you will have access to knowledge, tools and self-care techniques to help you make a real difference in your health.
Your nurse can work with you to schedule regular calls to discuss your health and make adjustments in your daily routine to help you live healthier and better manage your condition. You can also call your nurse when you need advice.
