Transition of Care refers to individuals that are in a treatment program at the time their coverage transitions from one provider network to another. Examples of medical conditions that may meet the guidelines for Transition of Care include but are not limited to:
If you are in a long-term treatment program like one listed above: Make sure your current physician is in our PPO Network. If they are, continue to see them and you will receive the In-Network benefit levels for their services.
If your physician is not in our PPO Network: You can apply to continue the treatment program with that physician. Please complete the Transition of Care form below.
