Blue Cross and Blue Shield of Illinois (BCBSIL) along with national and local thought leaders gathered in Chicago on June 13, 2018, to discuss readmission rates at the Blue University event. The keynote speaker, Suzanne E. Mitchell, M.D., MS, a board-certified palliative care and family physician, as well as a panel of medical experts and BCBSIL personnel, shared their experiences and proven results on reducing readmission rates.

To continue the discussion, on Sept. 18, 2018, Suzanne Mitchell, will lead an interactive live webcast titled, Strategies and Tools for Improving Transitions of Care. As a follow up to her June presentation, Dr. Mitchell will go into more depth on a variety of topics including:

  • Project RED – reducing readmissions through the Re-Engineered Discharge research group, which develops and tests strategies to improve hospital discharge processes
  • Project BOOST® – Better Outcomes by Optimizing Safe Transitions
