Infusion Site of Care Management Program to Target Select BCBSIL Members

Blue Cross and Blue Shield of Illinois (BCBSIL) is working with Coram® CVS/specialty Infusion Services (Coram) to help identify members* who may benefit from Coram’s infusion care services. These services can be offered in the member’s home, at a Coram infusion suite or select CVS/pharmacy® locations.
Members who have been identified as possibly benefiting from these services will receive an awareness letter, followed by a phone call from a Coram representative. If you are a prescriber for an identified member, you also may receive an awareness letter and follow-up phone call from a Coram representative. The representative will discuss whether or not this alternative site of care option is beneficial for your patient and can help you with the transition process.
The Coram infusion site of care management program can offer your patient:
- Savings – helps reduce member out-of-pocket costs (copay/coinsurance amounts)
- Convenience – offers flexible scheduling, privacy or a location near the members’ home or work
- Care and Experience – centralized patient admissions, a nurse coordination team and patient advocates to help ensure your patient gets the appropriate infusion care they need
If you have any questions about this program, please call the Pharmacy Program number on your patient’s member ID card. Or, contact Coram toll-free at 844-242-0677 weekdays, from 7 a.m. to 8 p.m., CT. If your patient reaches out to you with questions about their pharmacy benefits, please advise them to contact the Pharmacy Program number on their member ID card.
*This infusion site of care management program is for members with an individual benefit plan offered on/off the Illinois Health Insurance Marketplace and who have been receiving infusion therapy at an outpatient facility.
