Omada®

Omada is a personalized program that surrounds you with the human support and digital tools to help you lose weight, improve your overall health, and reduce your risk for type 2 diabetes and heart disease.

Eligible participants receive:

  • A professional health coach
  • Weekly interactive lessons
  • Insightful health metrics

Omada is a separate company that provides services for Blue Cross and Blue Shield of Illinois. Omada is solely responsible for the products and services it provides.

Hinge Health

Hinge Health is an innovative digital program for chronic back, knee, hip, shoulder, and neck pain. Their digital resources can help you easily manage your overall musculoskeletal health.

Hinge Health features:

  • A tablet and wearable sensors
  • Unlimited one-on-one health coaching
  • Personalized exercise therapy

Hinge Health is an independent company that provides an online musculoskeletal program for Blue Cross and Blue Shield of Illinois. Hinge Health is solely responsible for the products and services that it provides.

WondrTM

Wondr is a skills-based digital weight loss program that teaches you how to enjoy the foods you love to improve your overall health.

Wondr includes:

  • Counseling for skill building, reinforcement and maintenance
  • Self-paced, informative, online video sessions
  • Available health coaches and an online community for social support

Wondr HealthTM, formerly Naturally Slim, is an independent company that has contracted with Blue Cross and Blue Shield of Illinois (BCBSIL) to provide metabolic syndrome reduction program for members with coverage through BCBSIL.

Livongo®

Livongo is a health benefit that helps make managing diabetes and hypertension easier for you and your family members with an integrated experience that addresses the whole person no matter how many conditions they have.

Members can access Livongo’s:

  • Real-time, personalized tips with each blood glucose check
  • Unlimited strip reordering right from your meter
  • Automatic uploads means no more paper log books
  • Easy remote monitoring via wireless-connected blood pressure cuff
  • Expert guidance for reviewing current medications and facilitating conversations with providers and pharmacists
  • 24/7 coaching in nutrition and weight, stress and blood pressure management

Livongo for Diabetes
