Yes. In fact, it's likely that your doctor is already a member of this network and you’ll automatically receive network discounts. To confirm that your doctor is a member of the Blue Cross and Blue Shield PPO network, call the Customer Service Center at Blue Cross and Blue Shield at (800) 5-FLY-UAL or (800)-535-9825, or consult our list of Doctors and Hospitals. If your doctor is not a member of a Blue Cross and Blue Shield PPO network, you may continue seeing that doctor but you may not receive discounted services.
How will ongoing treatment for a medical condition be affected by changing to the PPO Medical Plan?
Let’s say you are receiving treatment on January 1, 2005 for a medical condition (for example, chemotherapy or treatment for an allergy). The first thing you need to do is show your ID card from Blue Cross and Blue Shield. Chances are, your doctor is already a member of the Blue Cross and Blue Shield PPO network. To make sure, give your doctor a physician notification card so the doctor is aware that Blue Cross and Blue Shield now administers your benefit plan. Also keep in mind that when you receive pre-certified inpatient care at a Blue Cross PPO network hospital, you’ll continue to receive the $200 incentive payment from United Airlines. Why do I need to carry my identification card?
You must show your identification card so your doctor knows you are entitled to receive medical and prescription drug benefits under the United Airlines PPO Medical Plan. Because the card shows your name, group number and file number along with the Blue Cross and Blue Shield symbols, it's personalized for you alone. The card also contains instructions on how to file claims for prescription drug benefits. Showing the card ensures that you receive any applicable discount and that your claims are processed quickly and accurately.

Will my family members receive their own identification cards?
If you have single coverage, you’ll receive one identification card. If you have family coverage, you’ll receive two cards. To request more cards, call Blue Cross and Blue Shield Customer Service at (800) 5-FLY-UAL or (800) 535-9825.

A directory of PPO network doctors and hospitals will be sent to you within two weeks. You may also search online list of local and national Doctors and Hospitals.
