If I join the Traditional Medical Plan, will I be able to continue using the doctor I have now?
Yes. In fact, it’s likely that your doctor is already a member of this network and you’ll automatically receive network discounts. To confirm that your doctor is a member of the Blue Cross and Blue Shield PPO network, call the Customer Service Center at Blue Cross and Blue Shield at 1-800-5-FLY-UAL (1-800-535-9825), or consult our Provider Finder® tool. If your doctor is not a member of a Blue Cross and Blue Shield PPO network, you may continue seeing that doctor but you may not receive discounted services.

How will ongoing treatment for a medical condition be affected by changing to the Traditional Medical Plan?
Let’s say you are receiving treatment on January 1, 2002 for a medical condition (for example, chemotherapy or treatment for an allergy). The first thing you need to do is show your Identification card from Blue Cross and Blue Shield. Chances are, your provider is already a member of the Blue Cross and Blue Shield PPO network. To make sure, give your provider a physician notification card so the provider is aware that Blue Cross and Blue Shield now administers your benefits plan. Also keep in mind that when you receive pre-certified inpatient care at a Blue Cross PPO network hospital, you’ll continue to receive the $200 incentive payment from United Airlines. Why do I need to carry my identification card?
You must show your identification card so your provider knows you are entitled to receive medical and prescription drug benefits under the United Airlines Traditional Medical Plan. Because the card shows your name, group number and file number along with the Blue Cross and Blue Shield symbols, it's personalized for you alone. The card also contains instructions on how to file claims for prescription drug benefits. Showing the card ensures that you receive any applicable discount and that your claims are processed quickly and accurately.

Will my family members receive their own identification cards?
If you have single coverage, you’ll receive one identification card. If you have family coverage, you’ll receive two cards. To request more cards, call Blue Cross and Blue Shield Customer Service at 1-800-5-FLY-UAL (1-800-535-9825).

How can I receive a directory of PPO network providers?
Just call Blue Cross and Blue Shield Customer Service at 1-800-5-FLY-UAL (1-800-535-9825). A directory of PPO network providers will be sent to you within two weeks. You may also use our Provider Finder® tool on line. The Provider Search tool allows you to perform different queries on the providers in the PPO network across the country. To request a personalized directory of network providers in your immediate geographic area, call GeoAccess toll-free at 1-888-821-1188.
