Objectives and Goals
The objectives of MMAI, ICP and FHP are to support integrated, person-centered care that promotes member independence with strengthened LTSS and improved transitions between care settings. MMAI, ICP and FHP are committed to working with providers and members to achieve a high level of satisfaction in the delivery of quality health care services.
MMAI, ICP and FHP are designed to help achieve the following goals:
- Improve care delivery, coordination and utilization of community-based services
- Increase member access to high quality care and community-based services
- Establish a single point of accountability for delivery and coordination of primary, acute, behavioral health and LTSS
An additional goal of the FHP is breaking down the financial, cultural and linguistic barriers preventing low-income families and individuals from accessing health care.
