THIN INC. (IL) SUBSCRIPTION TERMS

These are the subscription terms for THIN Inc. (IL) between the party whose name appears on the THIN Inc. (IL) Enrollment Rider form and Health Care Services Corporation ("HCSC"), Chicago, IL, and Metropolitan Chicago Healthcare Council ("MCHC"), Chicago, IL. In this document, "you" or "your" refers to the individual or entity who signs this Agreement, and "we", "us", or "our" refers to HCSC/MCHC.

BENEFITS OF THIN Inc. (IL)

The various benefits and services offered by THIN Inc. (IL) are fully explained in the "Benefits and Options" (29KB) brochure.

TERMS AND CONDITIONS OF SERVICE

These terms shall take effect when we have approved your request for subscription services and activated your submitter identification number. Important: Regardless of the subscription level and term you select, (if appropriate) by signing THIN Inc. (IL) Enrollment Rider Form, you agree to a subscription period with us. The subscription period starts when these terms take effect. Regardless of the level of subscription you have selected, these terms shall remain in effect throughout the defined subscription period.

The subscription level you have chosen includes certain services to be used throughout the subscription term. If you actually use fewer than the offered services, you will not receive any credit for the unused services. Services not used in one subscription term are not carried over to the next term and are forfeited.

PAYMENT RESPONSIBILITIES:

Payment of your subscription fee is due on the date indicated on the bill. Balances which remain unpaid on the next billing date shall accrue a late charge of one and one-half percent (1-1/2%) of the balance due per month, or the highest rate permitted by law, whichever is less.

If you do not pay any charges owed to us or if you violate any of these terms, in addition to any and all rights and remedies available to us at law or equity, we have the right to discontinue or restrict any and/or all of your subscription services, either temporarily or permanently. In either case, we shall incur no liability whatsoever. However, before restricting or discontinuing your subscription for non-payment, we will give you reasonable notice of our intention. If we refer your account to an attorney or agency for collection, you agree to pay our actual collection costs, including court costs and reasonable attorneys’ fees, unless prohibited by law.

For each check returned unpaid, we will impose a charge of $25 or the highest amount that is allowed by law, whichever is less.

RENEWALS:

At the end of the current term, your subscription will be automatically renewed for successive thirty (30) day terms at the current level.

ASSUMPTION OF SERVICE:

If at any time you elect to have your submitter identification number assumed by some other person or entity, you will still remain responsible for any and all charges incurred under these terms up to the date your number is assumed. No assumption of network service will be effective without HCSC/MCHC’s consent.

CHANGES/MODIFICATIONS:

HCSC/MCHC can modify or amend the terms at any time by sending you notice in your statement/bill. If you do not agree to the changes made, you must notify us in writing to cancel your subscription within thirty (30) days of our notice to you and pay all outstanding charges owed to HCSC/MCHC, otherwise you will be conclusively deemed to have agreed to the changes described in our notice.

REINSTATEMENT FEE:

A Reinstatement Fee will apply if your service is disconnected.
