Effective August 21, 2008, Centegra Northern Illinois Medical Center and Centegra Memorial Medical Center adopted the following new assumed names:

• Centegra Memorial Medical Center will do business as:
  Centegra Hospital – Woodstock

• Centegra Memorial Medical Center, South Street will do business as:
  Centegra Specialty Hospital – Woodstock, South Street

• Centegra Northern Illinois Medical Center will do business as:
  Centegra Hospital – McHenry 

These name changes do not affect members who use hospital, ancillary service facilities and affiliated physicians in the Centegra network. Members will continue to receive treatment and services without interruption. Since there is no change in Tax Identification Number or NPI, there should be no disruption to the claims process.

We are in the process of revising Centegra's listings in the Provider Finder^® section of our Web site. It is important to note that, while individual physicians affiliated with Centegra can still be found via a name search within our online Provider Finder, we are still in the process of changing the associated hospital facility name. Please be advised that, in the interim, members may still need to search using the former facility name.
