Beginning November 1, 2008, all HMO Illinois and BlueAdvantageSM HMO members who have access to Blue Care Connection (BCC) will be eligible for telephonic wellness coaching for the Weight Management and Tobacco Cessation programs. There will be no price increase, in 2008, for this new feature.
Members will be identified as candidates for the Weight Management and Tobacco Cessation telephonic wellness coaching programs through:
- Worksite Wellness Event
- Health Risk Assessment
- Self-refer by calling Customer Service using the phone number on the back of their member ID card
Once members are identified (as needing assistance), they will receive outreach regarding the BCC programs available to them according to their "readiness to change." Wellness Coaches will call members to provide personal assistance with goal-setting and periodic progress checks for those who have the greatest need and desire to make a change in their health.
The online Smoking Cessation and Weight Management programs can also assist your client's members who want to quit smoking and/or lose weight, helping them to reach their goals through a variety of resources. To participate in these programs, your employees log in to Blue Access® for Members, click on the Personal Health Manager and select either the Stop Smoking or Weight Loss links. They can earn Blue Points for participating in these online programs*.
