Reminder about PPO and HMO Directory Post
Cards
To help reduce administrative costs, please encourage
your groups to use the PPO and HMO Directory Post
Cards if their employees need a directory.
The PPO postcard (EB4693.0401 Generic Directory
Pscd) and the HMO postcard (EB5273.0303 IL HMO Directory
Pscd) can be ordered by calling (866) 726-2767.
The HMO postcard gives the member a choice of requesting
an HMO Illinois or BlueAdvantageSM HMO directory.
All postcards are returned directly to a fulfillment
service, which will mail a directory to the member
requesting it.
