Blue Cross Community MMAI (Medicare-Medicaid Plan)^SM includes eye benefits. You do not need a referral to see an eye doctor in the plan network. Visit the Davis Vision^® website to find vision providers.

What Services Are Covered?

Some of the vision services covered include:

• Routine eye exams
• Eye glasses and replacement lenses
• Contact lenses
• $130 towards upgraded frames every 2 years

What Services Are Not Covered?

The plan does not cover:

• Contact lens insurance
• Laser vision correction
