Important information regarding the physician fax form:

• Orders not faxed from a licensed physician’s office will not be processed.
  
• Complete every field in the Member Section of the Physician Fax form to make sure that your order is filled properly.
  
• Prescriptions for controlled substances cannot be filled from a faxed prescription. You must mail the original prescription signed by your doctor to the home delivery pharmacy.
