This program will help you:
What can you expect from a Comprehensive Medication Review (CMR) and a Targeted Medication Review (TMR)?
For a CMR:
You will be asked to have all your medication with you. A trained MTM nurse or pharmacist will review each of them, including how much and how often you take them. The review usually takes about 25–45 minutes. After the meeting, you will receive a summary of the discussion, a Medication Action Plan, and a Personal Medication List (PML). A sample of the PML is included at the end of this document.
For a TMR:
Every few months a clinician from the MTM program will review specific medication issues to make sure you are getting the most benefit from your medications. Your provider may be contacted if a drug therapy problem is identified, or if there is a potential medication cost–savings for you.
It's easy to get started:
If you meet the criteria below, you will be automatically enrolled in the MTM Program. You will get a letter that tells you how to set up a CMR appointment.
There is no added cost to you for this program. You may leave the MTM program at any time. You may also choose to take part in only those individual MTM services you find valuable. Your choice to take part – or not – has no effect on your insurance benefit.
You are eligible to participate if you meet the following criteria:
1. Have three or more of the conditions listed below:
2. Take six or more prescriptions, AND
3. Spend more than $3,017 on medication in 2014
