This process enables you to request a Group Health Quote. Once you submit the information, we will respond to you within two business days.

To receive a Group Health Quote, you must provide demographic information about your business and employees. At Blue Cross and Blue Shield of Illinois (BCBSIL), a division of Health Care Service Corporation, we understand how much you value your privacy. You can be assured that we are committed to protecting the information you provide over the Internet in the same way that we protect it when you provide by phone, in person or through the mail.

To qualify for a Group Health Quote, the following conditions must be met:

• Your company must be domiciled in Illinois
  
• At least 51% of all your eligible employees must reside in Illinois. No more than 24% may reside in Wisconsin.
  
• All employees working 30 hours or more per week must be included on your quote, unless they waive coverage because they are covered under their spouse's group health plan. A minimum of 50% of employees working 30 hours or more per week must participate on the group health plan.
