Process for BlueEdge Individual HSA Prospects
BlueEdge Plan & Mellon HSA Presented Together by Producer |

| 1. Producer has option to present Mellon HSA product at time of BlueEdge Individual HSA application (if so, include Mellon brochure |
2. Prospect completes and returns Request for HSA form along with health plan application |
BlueEdge Plan is Activated |
3. Once policy is activated, BCBSIL sends Mellon a file of HSA enrollment information (name, address, SSN#) |
Mellon HSA Welcome Kit Mailed |
4. Mellon verifies the identity of the individual, assigns HSA account number and sends the new HSA customer a welcome kit including a letter, signature card, beneficiary form, deposit ticket and disclosure statement. The member provides additional identification and enrollment information and signs the signature card. |
Mellon Account Established |
5. The member mails the completed signature card to Mellon in a postage paid envelope that is provided |
6. Mellon opens the HSA, posts check, if applicable, and sends a debit card and check book (with deposit tickets) to the member |
Note: Mellon requires enrollment file from BCBSIL in order to establish an HSA.
