When you receive care from a PPO or HMO doctor or hospital, you do not need to pay up-front for your care (except copayment). Network doctors and hospitals will file claims on your behalf directly with Blue Cross and Blue Shield of Illinois. You won't have to complete a claim form unless you receive care from a doctor or a hospital that is not part of the PPO or HMO networks.
