International Coverage
When you travel outside the United States and need medical assistance services, call 800-810-2583 or call collect to 804-673-1177 for information. Blue Cross and Blue Shield has contracts with doctors and hospitals in more than 200 countries.
Care Management Incentive
You or your adult dependent (18 or older) may be eligible to earn up to $200 in gift card incentives if you enroll in one of Abbott's four national plans for 2012 and have one or more of these conditions (based on the health option you choose):
- Asthma
- Chronic Obstructive Pulmonary Disorder (COPD)
- Coronary Artery Disease (CAD)
- Congestive Heart Failure (CHF)
- Diabetes
How it works
If you or your adult dependent (age 18 or older) are identified as having an eligible condition, you will be contacted by a care management nurse. The nurse can help you set and work toward realistic health goals to improve or maintain your health. The care management nurse will explain how you can qualify to receive up to $200 in gift card incentives. Beginning January 1, you may take the initiative to participate, or just call Blue Cross and Blue Shield to learn more at 800-671-1210.
Prescription Drug Information
As a BCBS member, your prescription drug benefits are administered by Caremark. View a list of the Abbott drugs that are covered 100% for Abbott employees
, effective 1/1/2012. If you have questions about your prescription benefits, please contact a Caremark Customer Care representative toll-free at 866-293-8009. Caremark automated services are available 24 hours a day, 7 days a week.

Mental Health and Substance Abuse Coverage Changes
- Mental Health Networks — Your mental health and substance abuse benefits are administered by United Behavioral Health (UBH). To find a mental health provider in the UBH network, visit Live and Work Well
(access code: abbott) or call 866-808-5075. - Pre-certification required — You must pre-certify all inpatient and most outpatient mental health services. We strongly encourage you to call UBH before receiving any mental health or substance abuse services to ensure coverage and avoid unnecessary out-of-pocket expenses.
