  • Review information regarding Retail Health Centers (RHCs), also known as store-based health clinics. Learn more about RHCs.

Note: When searching in the U.S., Wisconsin members should select WI as their state of residence and Blue Preferred POS as their health plan when prompted, not PPO. When searching for doctors and hospitals outside of Wisconsin you should select the state you are visiting and PPO as the health plan.

Prescription Drug Information

Behavioral Health Benefits
Your behavioral health (mental health and substance abuse) benefits are administered by United Behavioral Health (UBH) for the following plans:

• BCBS PPO Plus (including Altavista and Hawaii)
• BCBS of Wisconsin Blue Preferred POS

Claims prior to January 1, 2010 will need to be submitted to Magellan Health Services.

Pre-certification required
You must pre-certify all inpatient and most outpatient mental health services. Benefits for the following services will not be paid if not pre-certified: Autism treatment, Psychological testing, Biofeedback, Electroshock therapy, and Hypnosis. We strongly encourage you to call UBH at (866) 808-5075 before receiving any mental health or substance abuse services to ensure coverage and avoid unnecessary out-of-pocket expenses.
