Abbott's Employee Assistance Program (EAP) Through Ceridian
As an active Abbott employee or pre-65 retiree, you and your family are eligible for EAP services administered through Ceridian. The EAP provides up to six short-term counseling sessions at no cost to you. Professional EAP counselors can help you find solutions to:
- Relationship and family difficulties
- Stress
- Depression
- Alcohol or substance abuse
- Parenting issues
- Grief and loss
- Life transitions

Behavioral Health Care Benefits Available Through United Behavioral Health (UBH)
Effective January 1, 2010, behavioral health care (mental health and substance abuse) benefits are administered by United Behavioral Health (UBH) for the following plans:
- BCBS PPO Plus (including Altavista and Hawaii)
- BCBS of Wisconsin Blue Preferred POS
Claims prior to January 1, 2010 will need to be submitted to Magellan Health Services.
Behavioral health benefits give you access to in-person support for a wide range of concerns, including:
- Alcohol abuse and substance abuse
- Depression
- Anxiety
- Eating disorders
- Relationship problems
Pre-certification required
You must pre-certify all inpatient and most outpatient mental health services. Benefits for the following services will not be paid if not pre-certified: Autism treatment, Psychological testing, Biofeedback, Electroshock therapy, and Hypnosis. We strongly encourage you to call UBH at (866) 808-5075 before receiving any mental health or substance abuse services to ensure coverage and avoid unnecessary out-of-pocket expenses.
