Offical Rules

From June 19, 2008, until September 26, 2008 (the “Program Period”), Blue Cross and Blue Shield of Illinois, Blue Cross and Blue Shield of New Mexico, Blue Cross and Blue Shield of Oklahoma, and Blue Cross and Blue Shield of Texas, Divisions of Health Care Service Corporation, A Mutual Legal Reserve Company, will purchase one tree to be planted in Illinois, New Mexico, Oklahoma, or Texas for every 2,500 members who log in to Blue Access® for Members and choose to receive their Claim Statements (EOBs) online up to a maximum expenditure of $25,000.
 
Blue Cross and Blue Shield of Illinois is a Division of Health Care Service Corporation, a Mutual Legal Reserve Company, an Independent Licensee of the Blue Cross and Blue Shield Association.
 
Eligibility: This promotion is open only to legal residents, age 18 or older, who are current members of Blue Cross and Blue Shield of Illinois, Blue Cross and Blue Shield of New Mexico, Blue Cross and Blue Shield of Oklahoma, and Blue Cross and Blue Shield of Texas, Divisions of Health Care Service Corporation, a Mutual Legal Reserve Company, as of June 1, 2008. Eligible members must register or be registered to use the Blue Access for Members Web site and elect to receive their Claim Statements (EOBs) online instead of U.S. Mail during the Program Period. Subscriber dependents and current members who previously opted to receive their EOBs online are not eligible. Void where prohibited.
